CONSENTEMENT POUR LA PARTICIPATION AU PROJET PILOTE

Titre du projet : REACH Soins de soutien communautaires contre le cancer : un projet pilote

Description du projet : L'objectif principal du projet pilote REACH (Resources, Encouragement & Accessible Cancer Support for Healing) est d'établir des voies de référence ciblées au sein des communautés où les personnes ayant reçu un diagnostic de cancer peuvent se heurter à de multiples obstacles pour accéder à des soins de soutien vitaux à la suite de leur diagnostic.

En plus de fournir un soutien ciblé, nous visons à apprendre et à comprendre continuellement ce que les individus veulent et ce dont ils ont besoin pour leurs soins de soutien continus contre le cancer. Cela nous aidera à adapter et à personnaliser nos programmes et nos ressources en conséquence au cours des mois et des années à venir.

Objectif des commentaires : Je consens de participer au projet pilote mentionné ci-dessus. Je comprends que ma participation implique la collecte d'informations sur ma satisfaction à l'égard du programme aux fins suivantes :

1. Évaluer l'impact du projet pilote sur mon bien-être.

2. Recueillir des commentaires pour améliorer les services à venir au sein de la communauté.

Collecte et utilisation des commentaires :Je comprends que, dans le cadre de ce projet pilote, les commentaires seront collectés au début et à la fin du projet. Cela impliquera :

1. Une courte enquête au début du projet pilote.

2. Une courte enquête à la fin du projet piloted 6 mois.

Rémunération :

Je comprends que je recevrai une carte-cadeau de 25 $ pour le temps que j'ai consacré à répondre à l'enquête initial et une carte-cadeau supplémentaire de 50 $ pour le temps passé à répondre à l'enquête final et à l'entretien.

Confidentialité :

Je comprends que tous les renseignements que je partage resteront confidentielles et sécurisées. Mes renseignements seront rendus anonymes pour les évaluateurs du projet.

Retrait du projet pilote :

CONSENT FOR PARTICIPATION IN PILOT PROJECT

Project Title: REACH Community-based Supportive Cancer Care: A Pilot Project

Project Description: The primary objective of the REACH (Resources, Encouragement & Accessible Cancer Support for Healing) pilot project is to establish targeted referral pathways from within communities where individuals with a cancer diagnosis may encounter multiple barriers to accessing vital supportive care following their diagnosis.

In addition to providing targeted support, we aim to continually learn and understand what individuals want and need for their ongoing supportive cancer care. This will help us to adapt and customize our programs and resources accordingly over the coming months and years.

Purpose of Feedback: I consent to participating in the above-mentioned pilot project. I understand that my participation involves the collection of information about my satisfaction with the program for the following purposes:

1. To assess the impact of the pilot project on my wellbeing.

2. To gather feedback to improve future services in the community.

Feedback collection and use: I understand that as part of this pilot project feedback will collected at the beginning and the end of the pilot project. This will involve:

1. Short survey at the beginning of the pilot project.

2. Short survey at the end of the 6-month pilot.

Compensation:

I understand that I will receive a $25 gift card in compensation for my time to complete the initial survey and a further $50 gift card for my time to complete the final survey and interview.

Confidentiality:

I understand that all information I share will be kept confidential and secure. My information will be made anonymous to the evaluators of the project.

Withdrawal from the Pilot Project

I have the right to withdraw my consent and discontinue participation in the pilot project at any time without any negative consequences. If I choose to withdraw, I understand that any feedback collected to date will be used as outlined above.
